I certify that I am the owner of the pet(s) stated above. I hereby grant permission to Carolina Canine Consulting to act on my behalf and in my pet's best interest by obtaining veterinary or other care at my expense, if deemed necessary, for illness or injury occurring to this pet(s). I further agree to pay for all veterinary and other necessary services incurred by and for my pet(s) during its stay and under the direct care and supervision of Carolina Canine Consulting whether in the privacy of thier own home or in public. Carolina Canine Consulting strives to provide exceptional, professional care to prevent injury or illness to my pet(s). However, in the event of illness or injury to my pet(s), Carolina Canine Consulting shall not be held liable for any injury or damages arising out of or related to such injury or illness.
